Who we are

Accessible Support Planning LLC is a Minnesota-based CDCS (Consumer Directed Community Supports) Support Planning agency. We help individuals with disabilities, older adults, and their families navigate long-term support services with confidence.

We guide people through every step, from developing person-centered support plans to understanding budgets, goals, and available resources. Our mission is compassionate, knowledgeable, personalized support that treats every individual with dignity and respect.

Every person deserves to live as independently as possible and to make choices about their own life. What makes us different is our commitment to lasting relationships, listening first, communicating clearly, and making a complex process easier to understand.

What is CDCS?

Consumer Directed Community Supports (CDCS) is a Minnesota program that gives eligible individuals more choice and control over the services and supports they receive. Your case manager can help determine your eligibility, and we help you turn that opportunity into a personalized, workable plan.

How Accessible Support Planning can help.

We focus on Minnesota CDCS Support Planning and stay with you throughout the year, not just at plan time. We start with two things every person deserves: a real conversation and a plan that reflects their life.

CDCS Support Planning

We develop person-centered support plans that reflect each individual's goals, strengths, needs, and preferences while meeting Minnesota CDCS requirements.

Annual Plan Renewals

We complete annual support plan updates to ensure services, budgets, and goals remain current and continue to meet each person's changing needs.

    When life changes, we help update existing support plans, budgets, and services to ensure continued support and compliance.

    We work with individuals and families to create CDCS budgets that maximize available funding while supporting independence, health, and safety.

I'm the founder and owner of Accessible Support Planning LLC. With over 10 years of experience in CDCS Support Planning and family members with disabilities, I understand the importance of compassionate, person-centered support.

Since founding Accessible Support Planning in 2020, I have been dedicated to helping individuals and families navigate Minnesota's CDCS program with confidence. My goal is to make the planning process simple, personalized, and focused on helping each person achieve greater independence and a higher quality of life.
